Maryland Department on Aging
NH Diversion Modernization Grant
The Maryland Department on Aging (MDoA) is working to implement a NH Diversion grant it received from Administration on Aging (AoA). The project consists of a variety of efforts, including developing targeting criteria for identifying individuals at high risk of institutional placement and developing reimbursement approaches.
The purpose of this web site is to facilitate sharing of information among Steering Committee and Workgroup members and provide greater transparency to the project. Below we have posted documents that pertain to each working group.
