Supporting Transitions for EPSDT Eligible Children in Foster Care with Mental Health Disorders in the District of Columbia

This page serves as a repository for materials created and used during the DC Department of Mental Health's EPSDT Portals Grant from the 2004 thru 2007 fiscal year.  The purpose of this project was to develop and establish an implementation plan and mechanisms for supporting foster care youth (18-21) during their transition from the child/adolescent mental health system to the adult mental health system in the District of Columbia.
